PHP ne peut pas télécharger des fichiers sur le serveur?

J'ai un fichier php qui télécharge des images comme des images jpeg et png dans un dossier nommé uploads qui est stocké sur le serveur apache et dans le même emplacement que le fichier php.

J'ai vérifié le code de la fois le HTML et le PHP et les deux semblent être parfaitement bien, mais chaque fois que j'essaie de télécharger un fichier que j'ai toujours un message d'erreur et le fichier ne veut pas se télécharger.

Il serait apprécié si quelqu'un avec plus d'expérience que moi peut regarder mon code et me dire pourquoi il se comporte de cette manière.
